MATIT requires that claims be submitted online through the member portal. Below are instructions on how to submit a claim to MATIT using its online system. The instructions are for property, auto, and liability claims.
1. Log into the MATIT Website
2. On MATIT’s homepage, select “Submit a Claim” select either CLC or Workers’ Compensation
When you select Workers’ Compensation you will be directed to the State Fund Mutual website. MATIT contracts with State Fund Mutual to administer workers’ compensation claims.
You are now at the Member Portal dashboard.
3. Enter your Member Name
4. Enter your Username and Password
5. Select the appropriate form to report the incident/loss/claim
Hover your arrow/mouse over each button for information (buttons: auto, property, liability, bond)
6. Enter information in the fields as required
Complete forms as much as possible. Required field have red asterisk (*). These fields must be completed or you cannot submit the claim.
7. After you have entered all required fields, upload supporting documents (i.e. summons and complaints, photographs, police and accident reports, damage estimates, etc.) to Optional Supporting Documents (hover your arrow/mouse for directions)
8. Select Completed located at the bottom of the form. If you do not click “Completed” the claim is not submitted to MATIT nor is it saved.
9. You will get a message indicating that your claim has been sent to MATIT.
10. MATIT will contact you with information regarding the claim.
